I need a contact database with fields for title, first name, last name, category, 2 address lines, city, state, zip, and phone number. Must have ability to print labels, envelopes and other forms in casual first last name format or with titles (Mr. & Mrs.). Requires flexible printing options by group, with/without titles, on various sizes & types of paper. Prefer Access database with VB menu system. I also need to have address data from 10 small Word docs imported into the database, approximately 750 total addresses. A sample of the doc layout is included and any questions should be answered before bidding. I would like to have it completed within 7 days. Feedback and suggestions appreciated. Source code must be included with finished product.
