Modify a form in Excel for report generation & Format the report results

A Synapse data warehouse contains a view called "Brass Tacks Budgets" which hold information about project cost transactions and different budgets against those jobs. We require a form to be built in Excel which will allow the user to select key parameters like: - Company - Job Number - Budget Type (Budgets available depends on what types of budgets are available for the Job) - Posting From / To Date, and cost codes. A button labeled "Fetch" on the form needs to be pressed to retrieve the report. When the user hits this button, in the background a connection to the synapse data warehouse should be created along with the query string with the selected parameters from the form. The report must be formatted as per the sample which will be provided. If the user wants to rerun the report with new parameters, they must click on the Icon in the top left to pull open the form. A “Unselect” button should allow all previous selection to be unselected and reset the form to its defaults.
