Web form possibly in acrobat(repost) (1442493)

I need? an online? application form that can be completed online by users? and then the whole complete? form? sent email address after completion I already have? the general gist of the form? done in adobe with the active fields, this can be used as a template and the contents and fields must all be included. The completed form MUST be then sent as a completed PDF document not just the field data, this has to be the case as the pdf forms are then saved as a single file in the applicants file. I have space on both linux based and windows based servers so you should be able to use the language of your choice.
