Currently have a PHP/MySQL database with a PHPWord document creator added to generate preset word forms. We currently are experiencing an error when we are adding new cases by our data entry team we cannot access the new cases to generate the needed documentation. See attached error and provide price and time frame to complete. In addition, the link from our database to the doc creator needs to be added to 4 more pages.
