Development of a mobile (Android and IOS) single php page with a form that stores the form output in a MySQL database. Access to php my admin will be provided upon acceptance. I will be able to modify the form fields myself. The form (source code) should be easy to customize. Forms fields should include: - emailaddress - a pull down menu - 3 photo upload fields (accessing phone camera) unique filename to be stored in the database and photo to be uploaded to folder on the server (can be left empty) - lattitude and longitude to be retrieved from the phones current gps possition - current date - current time The user can submit minimal 1 and maximum 3 photo's Submit button that creates a record in a my sql database.
