Description: I want an online form on my website ([login to view URL]) that clients have to fill in (I do key management of vacation apartments), the information is stored in a SQL database. After filling it in, the customer will get an email with this information and a link where he can update his form - after each update an email will be sent to the customer and myself. Upon request and/or update, this has to be approved by me, so the status is on "request" until I have "approved" it via admin, then the customer gets "request approved". (also when updating the form). On the form please have a check against SPAM bots. I myself must have an admin login and a web page where I see all requests, and where I can search, sort or filter on all fields. Fields needed: (to be filled in by the client) - Requestor name - First name requestor - Email requestor - Applicant phone number (with click-through to Whatapp) - Guest name - First name guest - Guest phone number (with click through to Whatapp) - Number of adults - Number of children - Arrival date - Hour of arrival - Date of departure - Hour of departure - Arriving by: plane / car / other - Flight number - Airport transfer: yes/no - Check-in: yes/no - Check-out: yes/no - Cleaning: yes/no - Will guest pay cleaning fee directly to me? Yes/no - Additional important information or question: Additional fields (not visible to client) to be completed by me via admin only: - Unique number - Status: (request / approved / refused / finished ) o in case of a modification a mail is sent to the customer - Total price to be paid by the customer - Paid (yes/no/via subscription) - Note 1 by myself visible for the customer: - Note 2 by myself NOT visible to the customer: - Optional: Button to add appointment to my iPhone calendar. Via admin possibility to customize the text (header test) at the email templates. - New request (mail to customer & myself) - Update request (mail to customer & myself) - Status update (mail to customer) Under the header comes all the info from the database (entered by customer). Export of the database to excel via an export button (in admin). The website must be mobile viewable. ------- Price of this job (creating the program) incl. a 1 revision and minor adjustments (if needed) after 1 week of testing and a 2nd revision and minor adjustments (if needed) after 2 weeks. Payments: 10 % at acceptance of assignment 60 % at assignment ready and working 15 % after revision 1 15 % after revision 2
