Create user register/login section for my site with a appointment booking function

I have a salon business, website is static website - only HTML. No admin feature or backend, never needed it. I need someone who can develop me a simple, clean feature where people can REGISTER/LOGIN to my site. They can make bookings for their treatments. I offer 3 types of services. Some notes to know about - IMPORTANT NOTES: - Must sync with Mindbody [login to view URL] Login for Mindbody Admin at bottom. Website builder section for code snippets You can take a code from their website which integrates the calendar and booking process directly on your website, already checked it out and alot of people using it - Every service has a STUDIO and IN STUDIO feature to it. Before they choose their service, they will be asked in a dropdown where they want this to take place - Member section of a user should have a PREVIOUS APPOINTMENTS tab in it. When they click it they can re-order the same treatment without filling in personal information all over again - Due to having member functionality obviously that includes features like LOST PASSWORD for people also - Everytime appointment booked, email goes directly to the user with a welcome email about their appointment and also pre-post care instructions in the email (Link to a PDF). I have already designed that email - In total there are 3 types of service LIFESTYLE / PHOTOSHOOT and BRIDAL - People who sign up for the first time, they get a welcome message on the screen when they sign up Some of the main points, if you have worked with Mindbody before for another client even better. Any design material like forms will be provided to you in PSD format. I will give more information to peoples profiles I like.
