I have several [login to view URL] forms integrated into HTML webpages, CPANEL is eliminating this CGI protocol coding so I need to convert the forms to PHPMail. So looking for a PHP developer to help me. What I require is [login to view URL] file template based on sample forms with some general instructions to assist me in conversion. Note: the samples show "agree box Java scripting" to be replace with captcha.

Hello, Looks like the code you attached, specifically the form codes, are encrypted. No worries though. From the look of the form by viewing the HTML files you attached, I can recreate it and then send submission details using PHP.
