Update Look & Content of HTML Website/Script for XML feed to populate 2 page PDF applications

I have an existing website that will require some simplication of the look and the addition of around 25 similar application pages. The navigation works well and other pages about us are in place. The form applications are to be complete by me at [login to view URL]

After that I will take over to complete my applications.
This solution requires no programming of the forms. Since I will complete all forms, all you do is cut and paste the html provide by Wufoo. The data is handled on Wufoo's servers and I only recieve an email with the entered form info from Wufoo. I also can log on to a dashboard of my account which allow access to information.
The programming that is required is the XML feed for each form application filled out on line. From this I want a few different PDF applications to be populated. You will need to tell me how I can manage and access these populated PDF forms. Email, simple dashboard or both?

Also attached is a PDF application to be populated by XML feed. Different bonds require different parts of application to be filled out.
The application forms should integrate into the website which I want to have a simple look.
I will load the website on www dot suretyhub dot com in a few days but its not fully fuctional for the public so I don't want it up to long. Look is good though.
Programming should be in be in PHP / MYSQL as is Wufoo.
