Price Entry Form with Multiple Rows

Ok, Simply I need a form to enter a various products and prices... the form is like an invoice... A fixed fields 2 or 3 fields will be like date picker from, to and a select field for store name, and another select field for the market type the above fields will be before a table then there's a rows with the following fields: select field for product, select field for color, another 2 select fields will be set later, text field for the price then.. a botton for insert new item which will insert an new row to add more products... Note: the select fields selects from some tables not static fields... there's a free script you can work on but needs to modified to add the fixed fields before the table and some select fields in the rows...
