Flash 8 contact form woth PHP

I need a simple flash contact form to be finished and also to do a php file to go with it so i can receive the information that they have sent. check out the attached folder i'v started the work but need to be finished It need to contain a field for :: * Title * Name * Email Address * Phone number * Contact you by(check box "phone" and "email") * Subject * Comment i'v done most of the flash work just need to check and sort out the checkBox's need to do the php i'v look at some php but not really got it! but if you look at the attached file it will give you some idea what i'v done and whats left do to The name, email address and comment all need to be needed / inputted to send info, Maybe a start can go by the fields need to have input It needs to have a submit button and reset button if the user does not input the information in the fields needed, maybe the field can light up, suggesting it need to be filled in. Also, once the user has inputted all the info and hits the submit button, i need it to say that the message has been received, or a error note, if there can be no connection, so the user knows. Also, i need the email content to go to another email address, which i can include later. i need the .php file created for this to work as well!
