I need three online forms (in html) for my website: 1.) Application form (it will have max. 20 fields, it should be sent like email or uploaded into MySql database - this needs to be discussed) 2.) Contact form (with three custom fields, with working php file to send email to me and a copy of email to user, email notification should add time stamp and ip address of user) 3.) Newsletter subscription form (it should be sent like email or uploaded into MySql database - this needs to be discussed) I would need your quotes and I was having in mind forms like they are on wufoo dot com I already have css file and website template, so I would only need these forms...
