Brochure design

I need a 3 fold brochure designed. I have about 30-35 item pictures that need to be displayed on the inside of the brochure. The pictures need to have an item number and price. On the outside of the brochure, the front fold should have Name, Logo and contact info (which will be provided when the bid is accepted) The other two outer folds should have an order form that includes a column for item#, Quantity (QTY), Name, Address, Phone number, Size. If it can fit include a list of all the products with the item number.? The form needs to have space for 15-20 rows. Also include one row seperate from the from so that the owner of the form can write thier name, school, orders, and amount collected.
