We are in need of someone to design an online survey for us. We will be emailing companies asking them to fill out the survey questions and want them to visit our website to do so. The survey will be a maximum of 10 questions - some with yes/no answers, some with multiple choice and some with open answers. As well as the survey, we also need the designer to build in some sort of script to collate the results and sow statistics, i.e. x% answered A to question 1, etc. Please quote for the total job.

Greetings! A little about me: I am the lead designer/developer for the in-house development of intranet applications at my current employer. I use these projects as real-life examples of the processes, techniques and best practices for my students. I currently work as a technical instructor teaching SQL Server 2000-2005 database design/development/administration as well as Access and web design and development (XHTML, CSS, and PHP 5). Why Chose me: One of the projects I designed and developed for my employer was a survey for incoming students. I used PHP and a MySQL database for that project. The survey had all the different choice options you had mentioned. That project also required an aggregation of the data. I estimated delivery at 3 days. Day one is to gather all the requirements and begin the logical and flow design. Day two (which most likely will still be day one) to code it.
