When visible checkbox (id="iamaCheckbox") is CHECKED + a numerical value range between 0 and 49 is manually keyed into a visible form field ("TE") a.) Set value of hidden text form field value (id="PID") to AAAAAAAA11111 b.) Set value of hidden text form field value (id="BU") to SM (see ** below) c.) Set value of hidden text form field value (id="CN") to 111 (see ** below) d.) Set value of hidden text form field value (id="CID") to 111A (see ** below) e.)
