I need a WordPress website to work as a lead capture page like, but not as complex, as [login to view URL] It needs to be a responsive theme and be able to collect zip code and test it against valid NC zip codes, then gather county, name, phone, age date of birth, smoking or non-smoking, number of adults and dependents with sex to be quoted. All results must result in a response that 'your quote request requires that licensed agent contacts you'. If you agree to be contacted click confirm. From there I need a form emailed to me with each record. I'll also need terms and conditions, privacy and about us pages linked from the main landing page. I'd also like an embedded video or two, one on the main page and another page with news and videos. The videos may be Youtube Vimeo or a custom animation video yet to be created. You will install WordPress and the website on a hosting account that I control. I will retain all rights and control over the work when the job is completed.
