Facebook Contest App with mandatory "Share"

I would like this Facebook app to be exactly replicated for a new client : [login to view URL] - new design supplied. Five mandatory text fields (the form can't be submitted with one of them empty), and a "share" button to submit the form. What's important is that once you click "submit", the form then makes sure you actually shared the content on your wall before sending the form data. I need the new app installed on a free or cheap server (which I can provide if needed), and the Facebook app created afterwords. I can give you my Facebook credentials if necessary. Once the app is online, I need the source code and a quick guide written on how to deploy another app from the source code, with basic-to-moderate computing skills.
