build datagrid ui in winforms

I have been working in windows forms since 2003.I have checked the video as well as the attached pdf .. 1- Video shows one text column and 2 checkbox columns,but there are many fields in attached pdf. 2-Please explain how many columns will be there in datagrid. Hi! If task is only to add datagridview with that type of action and make Save button work - it should not be very complicated. Only extra information about database will be needed.
