Plugin that will display form after playing video

I need plugin that will work in next way:

1. From WP admin you will be able to specify

1.1 - YouTube/Vimeo URL;

1.2 - Add form or any text that will be SHOWN after video is played

1.3 - Possibility to play video automatically (autorun)

2. Generate shortcode from plugin, so you can place it on any post/page

Plugin image is attached (how it suppose to look).

Important:

Use player from [url removed, login to view] ([url removed, login to view]) (or any other HTML5 player that looks nice and display on mobile phones/tablets)

Important 2:

This HAS TO WORK on iPad/iPhone (iOS)

I have already worked on the code, got code working on the Windows browsers and Android... but I need it to work on iOS. I can help you, as I'm developer myself - but I'm having too much other projects.
