Short HTML JAVA page to produce retail barcode label

Project:
Write Java/html to create a retail store item label, printable from a browser. Label has 7 items (price, barcode/sku, title, author, location, current date, logo).
Currently working, but does not have the required fields and proper layout.
Barcode is generated automatically by server.
User submits form and the item label is generated. I need for the 7 items to appear as depicted in our "FUTURE" example. I cannot give you access to the server to test, but will be able to verify if your code is working properly, give you examples of the output, etc.
The code that I have access to is as follows, and generates the "CURRENT" image:
