E-mail a friend PHP or .NET

We need a PHP or .NET based "refer a friend" app and database. We need a form application that we can embed in a page that will allow people to send a message to as many as 10 friends. we need something similar to this: [url removed, login to view] We need to capture the sender's: First Name, Last Name, e-mail address, message (not to exceed 250 characters). Note we will need the ability to append our own text to the message (set via a variable in the form). We need to capture the recipient’s: First Name, Last Name, e-mail address We need a thank you page (set via a variable in the form) that the user is taken to upon submission of the form. The info needs to be e-mailed to us (set via a variable in the form), as well as each recipient. We need the ability to adjust a setting in the form to have the data go into a database as well if we want. We will not always need to use the database feature, so this should be a setting in the form that we can turn on or off. You must also provide the database. Please indicate how fast you can complete this in your bid. Optional features: (These are not required, but please indicate if your bid includes this or not) Web-based secure login database editing Web-based secure login database reporting Web-based secure login database exporting to tab-delimited text file. The chosen coder will set the form up on our server. ## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. Including installation on our server.

2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):

a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc.
