I need to have some text fields programmed into a PDF document with an application form that we already have designed. It's currently in PDF form and all the questions on this application needs to have text fields so that we can email this PDF file out and have people type their information into the text fields and then SAVE their answers to send back. Please let me see samples of this work...Or I will request that you show me a quick sample so I know you can handle this. See sample attached. The attached is just a sample...There will be 4-5 pages similar to this one that I need to have the text fields programmed into.
