We have been using a custom Access Database to record and report donations to our small charitable organization.

We would like to have our database structure to be modified to our needs as following:

DONORLIST FORM:

1. Address SHOULD BE UNIQUE by default.

2. Donor's Name can be duplicate (two or more persons with a same name)

DONATION FORM:

1. Be able to SEARCH BY STREET NUMBER (from Address field" instead of FullName as currently set). (to be modified)

2. Donation cannot be duplicate with same donor's name, donation date and check number (to be added)

3. Existing Donor will have a symbol shows "Existing Donor " (to be added).

4. User can print REPORT of each individual donation by CLICKING A BUTTON (to be added)

REPORT: Will be 2 pages totally. First page will be a custom HTML text (created from Microsoft Word) plus some basic information from a QUERY that includes:

1. Donor's Name and Address (current)

2. Check Number, Check Date, Amount, Donated For (current)

3. List of last 3 donations (to be added)

4. Year-To-Date TOTAL DONATIONS of selected Donor (to be added)

5. Up-To-Date TOTAL DONATIONS of selected Donor (to be added)

IMPORTANT NOTE:

1. We can import back our current data (donorlist & donations) into new modified database when completed.

2. We can re-modify again in the future this modified database when needed.
