The logo that needs to be used is attached in various formats. Please also use the Direct Connect logo attached as it is better quality.

I am reasonably happy with the layout of the attached form, but it has some imperfections with the formatting. Please make some changes where you can make it look better (content must remain the same)

############IMPORTANT#################
1) Form must be editable for the user (ie the user can enter text into the text boxes on their computer and check boxes etc for them to print)
2) I must receive an editable copy (ie a format where I can make changes to the form in the future and save a new PDF)
3) I have more forms for the winner to create after this contest
