The application should read http header,UA etc from excel and open web and populate data from another excel to a web form. Form tags should be configurable. pls share some similar examples. Lets discuss for details.

You are sure about only wanting a html form ? Because you are saying that input to the form must come from a excel . Your excel is mostly having tabular data ? ie multiple data entry of the same kind ? I need to understand your requirement better.
