There are two thing must be considered while choosing framework. Firstly security and secondly fulfilment of all requirements. And there are no framework which can generate forms on client's browser, plus open source framework might be deadly as lots of people can hack frameworks like WordPress, Joomla etc easily because of there code availability. I suggest to get a micro framework developed with ability to generate client side forms. I can do this, in fact I always prefer to develop micro framework for my client/projects with the functions they need within the application and i know few techniques to secure your application using token, username encryption etc and for file security you must have a physical firewall installed on to your hosting server (which most hosting companies do). I have done many PDF manipulation applications (like decoding them, reading them using Regular Expression, generating PDFs, etc) and would like to work on your project. As per my understanding you are in need to capture data in certain format which is say customisation by your end user for certain fields/ forms to be generated/embedded besides the normal forms. I presume you are not looking for any native app on devices but a good bootstrap or similar framework to achieve the goal of displaying across devices very efficiently. As per requirements for Client side we suggest 1) Angular JS based client or Bootstap based responsive site.
