small php custom code

Hello I am using wordpress and [login to view URL] I need to change the upload folder. The new upload folder must be in form/name of the company - date - time The first text box that the user has in formular is name of the company - field id 1 This code below upload the folder location but it gets the penultimate content value of this text box.
