Registration form with database and dashboard - 01/05/2018 18:06 EDT

Hi, I am looking for a simple registration form and dashboard. I have a linux hosting so better to work with php. If you think, you have a better option in windows, I can change my hosting. Registration form should include Turkish language characters. As this webpage does not support it, I had to change some of those characters. You may find a sample form in this link ([login to view URL]). I am writing them in English below for you to understand. If I can change these characters in the code, no worries, I can handle the turkish name conversion. Adiniz = Name (Short Answer is expected in this field up to 50 characters or DB limit if necessary) Eposta = Email (Email is expected in this field) Instagram hesabiniz = Instagram account (Short Answer is expected in this field up to 50 characters or DB limit if necessary) Yakin zamanda nereye seyahat planliyorsunuz ? = Where do you plan to travel soon? (Short Answer is expected in this field up to 50 characters or DB limit if necessary) Etkinlige hangi gun katilmak isterseniz ? = When do you want to join the meeting ? (User must choose minimum one option up to three in this field) Önkaydımı yap ! = Submit Pre-Registration Gray background box will be fine for the form. I attach a sample one. Same fonts and size like in the above link should be used in the form. All form submissions should be written to a database. An email should be also sent to me when the user submits the form. After user submits the form, we should be able to show a thank you message in turkish language (Bilgileriniz bize ulasti. Degerlendirmemiz tamamlaninca size geri donus yapacagiz. Ilginiz icin tesekkur ederiz.) for 10 seconds and forward to user to the homepage. This form should be prepared in a separate page like in "[login to view URL], or html" but I should be able to allocate it in the homepage as well. All the form sections are mandatory. Please put * sign at the bottom of the page to indicate * is mandatory. If a user forgets to give an input for these sections, a response should be displayed like in red fonts / Turkish language caharacters as below, you may try this in the test link above, If the user forgets to give input for "Adiniz = Name", response should be; Lütfen adinizi girin. If the user forgets to give input for "Eposta = Email", response should be; Lütfen eposta adresinizi girin. If the user forgets to give input for "Instagram hesabiniz" = Instagram account, response should be; Lutfen instagram hesabinizi girin. If the user forgets to give input for "Yakin zamanda nereye seyahat planlıyorsunuz ? = Where do you plan to travel soon?; response should be; Lutfen gitmeyi plandiginiz yeri giriniz. If the user forgets to give input for "Etkinlige hangi gun katilmak isterseniz ? = When do you want to join the meeting ?, response should be; Lutfen size uygun bir gun secin. As I have mentioned above; response includes some turkish characters which can not be seen even here. Form should support it. In the form section; instagram account example is displayed as "Örnek: factsandcities" Dashboard page should be password protected and I should be able change it. Dashboard page can be in English. In the dashboard area, date submitted section and above form entries are enough to display. I should be able to delete, read all, and export these entries to excel. New or unread submissions number can be highlighted at the top. There should be a basic filter to display read, unread and all submissions at the top. I am attaching an example of this page.
