Order form in Wordpress/Jotform

I'm looking for a medium-complexity order flow form for my business website, in the style of something like Jotform. It could be built on Jotform, a Wordpress plugin, or something similar you recommend. I am open to your suggestion as long as it meets the needs. I would like to provide you with the outline of elements and calculations/pricing and have the completed project in return. If you're familiar with ordering on Fiverr, this is sort of what I have in mind. Here is a brief outline: - This is an online order/price quote form - Customers will go through the form selecting options and variables - I want a live price quote displayed throughout the process so they can see how services affect the price. Descriptions of the services will be displayed as well and they can choose what they need. - Price calculations will be made in the background and could be affected by certain variables. For example, they will initially choose a length of episode for the job. A longer episode would mean some variables later would cost more. - At the end stage, they would see a final price and have the option to proceed with order. - The form does NOT need to collect payment, but it would need to collect name, email, country, province (only if within Canada) and provide a file upload which can send files to my Dropbox. - Output can be to email, Zapier, or both. If there's some capacity to someday allow users to login and/or save preferences in an account, that may be ideal. It's not important for now, but it may impact which platform we use. I could tackle this myself but I'd prefer to work with someone who is a little more experienced with creating these forms, and I simply provide you the data to add.
