A previous Freelancer designed a PHP web form for us that accepts some user entered values and writes those values to a MySQL table. We need this same application altered to accept slightly different values and insert those into a MySQL table. The MySQL table is already completed. We only need someone to adjust the current PHP form to work with the new data being entered. Currently Collected Data: 1. Quantity 2. Lot Number 3. Item Number New Data to be Collected: 1. Quantity 3. Lot Number 4. Expiration Date (This should be a date picker field) 5. Item Number (this should be a drop down selection populating from a MySQL table) As you can see, only the two changes are being made (#4 and #5) to the existing form.
