PHP-MYSQL Web Based Timesheet

Option 1: We need a programmer to create an online timesheet that will allow individual users (Employees) to log in (seperate logins/password per user) and fill in the sheet, print it out, and save the data in a database that can be accessed, retrived and modified online if necessary by the employee and/or administrator . This is a weekly timesheet so the user needs the ability to access the sheet daily until the week has ended or the sheet has been electronically signed/submitted by requesting that the employee enter the password. And changes to the sheet afer being submitted can be done, but will require text description for the change and resubmit of the electronic signature (i.e., request password). The Manager through an administrator interface could also make changes. The sheet will need administrative access as well so that the drop down fields on the sheet can be populated and changed as necessary by an admin (not the user). The timesheet should be programmed using PHP and MySQL. An attachment provides some additional details. The columns and rows will total at the left of the sheet and at the bottom like a spreadsheet. There also has to be to two more fields on the bottom with a signature line for the employee signature and the project manager's signature. These fields will appear on the printed sheet.
