help to set up a tree in oracle apex ASAP

I have a tree made in oracle apex. I need to add an option so from every directory in the tree I can create a new form based on the form I already have, and when I fill in the form it would be saved under that directory where it was created and can be accessed later through the tree. besides that that form will also show up in a separate report where it would show all the forms created in the tree and will show the location where it was created
