Create an Adobe form displaying fields depending on user input

I have a small business and use a form to set up customers on a small database. The form (see attached) is an Adobe Pro XI form. I need an Adobe form to be amended. Currently, all the fields display. However, I need the form to only display certain fields, depending on which option the person selects in section 1 (“please confirm purpose of this form”). I want the form to initially show just section 1, which has a drop-down box. The options the user can choose are: Add a new payee Amend an existing payee Confirm an existing payee (no changes) The fields in Section 2 (“source of information”) will then appear underneath – this will appear regardless of what option the user selected. The fields in Section 3 should only display if the user selected “Amend an existing payee” OR “Confirm an existing payee” The fields in Section 4 should only display if the user selected “Amend an existing payee” or “Add a new payee” The fields in Section 5 should appear regardless of what option the user selected in section 1.
