It is called multi step form, but it is tricky to get it done in squarespace because of its limited function, but the workaround I found in my last project was to create separate page for each steps and redirect the user to the next page once the user enter and submit a step. Depending on the number of questions you want and how you want the form data to be store and other functions, we will have to find a viable solution. In case you want to get things done easy and fast we can create a separate form in html somewhere and use the iframe to embed it in squarespace.
