Build a contact form for a Responsive Website and polish code.

I am on the process of developing a responsive website for my company. The website is pretty much ready, it has jQuery and java-script to do simple things like adjust the dimensions and layout responsively and dynamically (in jQuery) and auto scroll to sections. I do have a simple message box that is set up as a pop up and fades the stuff behind it. You job is to implement the popup screen to become responsive as the screen(dimensions) are reduced to a certain size. If the screen gets too small for the popup form to be filled/seeing/managed/etc, then the jQuery should adjust the popup section (width/height) to taque the whole screen so that it becomes easier to fill out the form in from a mobile device or tablet for example. Another goal of this project is to polish the code and organize IDs, Classes and styles in a professional and clear way. * ** I have built a simple form using openModal as an example of how the form you are supposed to build must look like. I want to you add to fields for the form: - Business - Phone number But I want the user to be able to submit the form even if the phone number section of the form is not filled out, i. e., phone number section must be optional. So the form should have the following sections: - Name - Business - Phone # - Email - Message The form should pop from either the header menu, or from the (contact us) button in the bottom, how ever, if the user clicks on the form from the header menu, then I want the screen to first scroll down towards the (contact us) button, and just then pops up with the form. Remember, if the user is interacting with the website using a small screen, then the pop up should go into fullscreen and the items inside the form should adjust accordingly so that the user can input and submit the information from his/her phone for example. I have a jQuery set up working already for the menu header to do this so you can probably do this with the already working jQuery.... You may write a script to collect/process the data from the form and send an email to our webservices email so that we can get the information submitted from our website. Once the user clicks on the "submit" button and the information inputed is correct, then the form should close and the user should receive a confirmation message saying that we will contact him shortly, then redirect the user to the main page. I want to see the code polished and clear to understand, most of the styles are in the [login to view URL], but the popup openModal css is in the [login to view URL] file. All the jQuery and JS that you will be working with for the responsive menu, popup, header and auto-scroll system is provided and the website is kind of working well now. However, I have not tested it yet and I know that we might run into some glitches before getting the final version of it tested and working.
