Need a form built in Excel or Word that uses a combination of text fields and drop down selections. The questions need to populate dynamically based on the answers provided. When the form is completed, it needs to have a button to email itself to a specific address.

There seems to be some confusion with the bidders so far about what "dynamic" means. In this case, I need a form that has drop down and check box responses to various questions, and what the following question is depends on the answer to the preceding question. In the attached draft, you will see several questions that have "Yes/No" as the possible answer, followed by "If No, why?". So in the dynamic form, if the answer was "Yes" the "If No, why?" question would never appear, but if the answer was "No", then it would show up and require an answer.

I am open to other mediums besides Word or Excel so long as they can be used offline, and have the ability to interface with Outlook to send an email with the results of the form.
