I am sending out an email that I need to have a URL (I think) that the recipient fills in the requested data, pushes "Submit" and that spreadsheet is saved. I really need you to tell me what I need to do this. I need this as soon as it can be done. This is a census gathering of basic information of about 2000 individual doctors and their office.
