Responsive Android and IOS intake form/submission application:

The application must pass all guidelines for placement into Google Play and IOS App stores (for a fee); must be clean coded with comments; easy to follow for another developer. The project will have basic validation functionality, a simple configuration screen for admin/owner of project (to populate drop-downs and turn on/off maximum 5 field validation points) for client side users. Project/Tool should have the client side questions/interface change in 5 languages (English<default>; Spanish; French; Hindi; Russian). The app will have a maximum of 10 total screens in workflow including admin setup page for client app (not considering changed language) and a maximum of 15 total input fields across application (so far less) and configurable drop-down values in app admin with dependencies on previous selections/answers to questions. Ability to take or attach multiple photos or documents, tag the file name / assign to a folder and attach with form submission in very simple way. This should be downloadable by my clients, logged in from admin panel and see their settings/configurations in the app, and be handed over for intake by clients. Ideal candidate will want long term relationship, be motivated, very well communicated in English and interest to work hard; quick self-learner.
