I need a design for a rectangular, folded piece of cardstock to go around packaged vanilla beans. I have uploaded a picture of a label I've been using that is reflective of what I've been looking for. Colors, font, picture, etc. do not have to be the same as the photo. The dimensions of the card stock are 2" width x 3" height x 0.5" depth when assembled, giving it 4 sides. Refer to the pictures attached.

On the front of 2"x3" photo in place and must have the following: "10 EXTRACT GRADE B MADAGASCAR VANILLA BEANS". 10pt type minimum necessary. Also on the front, as not to blend with the product description or obscure too much of a photo must read "INGREDIENTS: 100% WHOLE VANILLA BEAN PODS". 10pt type minimum necessary. See attached photo for an example.

On the back of 2"x3" must have the following: "PRODUCT OF INDONESIA". 6pt type. Also on the back "JL Gourmet Imports LLC 10851 SW CANTERBURY LN STE 103 TIGARD OR 97224". 6pt type. Also leave a small space for a UPC barcode dimensions 0.5"x1" in the lower left or right hand corner.

Native file please. "InDesign" preferred, PDF if not.
